The fax-on-demand forms ordering system maybe accessed 24 hours a day seven days a week
by calling 1 800 748-3676. The system allows callers to order forms from any touch
tone phone by entering a special five-digit code number assigned to the form. The
requested documents can be ordered from any touch tone phone and sent to any fax
machine/fax modem having a three-digit area code. The system will make several
attempts to transmit the requested documents if the receiving machine is busy. Most
faxes are sent within five minutes, however, because the system can receive requests
faster than it can send them, it could take more than an hour during heavy demand periods
for a fax to be transmitted.
