Download from the Web	You can get New York State's current and prior year (to 1985) forms by accessing our Web site. Forms are available in regular PDF (Portable Document Format) or as fill-in forms (for selected forms from 1999 to current). Find forms online. Note: Adobe Reader (or Acrobat) is required to view and print PDF files. See Adobe Reader - Requirements and Download Information

CD-ROM format	You can purchase 2009 Publication 352-CD for $20 per CD; the CD contains a large selection of income, corporation, and withholding tax forms (prior year CDs are available through 2004). The 2010 CD will only include income tax and is expected to be available by mid-January. To purchase the CD you may: - Print out the CD order form and mail it in with proper payment - Have the CD order form mailed to you by calling (518) 457-5431 and requesting Form OSB-63, Order Blank for Publication 352-CD - Have the CD order form faxed to you by calling 1 800 748-3676 and using fax code “13016”
